MBA VEBA (Voluntary Employees Beneficiary Association) offers a full range of employee benefits, provided by the only banker-owned, nonprofit trust in Missouri. MBA VEBA is dedicated to giving banks control over benefit costs. MBA VEBA uses Great West, PHCS, HealthLink, Midlands Choice and Novasys networks. The pharmacy network is Medco.

Premier Plan - click here for detailed information
This plan has 85 percent in-network coinsurance, 70 percent out-of-network coinsurance, $400 individual deductible, and $800 family deductible.

PPO Plan - click here for detailed information
This plan has $20 encounter fee for primary care physicans and $40 encounter fee for specialists, 80 percent in-network coinsurance, 60 percent out-of-network coinsurance, $700 individual deductible, and $1400 family deductible.

High-Deductible Health Plan - click here for detailed information
This plan has a $2,000 single coverage deductible, $4,000 family coverage deductible, 80% coinsurance in-network and 60% coinsurance out-of-network.  This plan has coinsurance for prescription coverage. Mental/Nervous claims are not covered.

Group Dental Plan - click here for detailed information
This plan has 100 percent coinsurance with no deductible for preventive care, 80 percent coinsurance along with $25 individual deductible and $75 family deductible for non-preventive dental care, and $1500 lifetime orthodontic benefit per person.  VEBA offers two plans - a $1000 maximum benefit coverage per person per year or a $2000 maximum benefit coverage per person per year.

Group Vision Plan - click here for detailed information
This plan has a $10 copay for comprehensive vision exam once every 12 months, a $25 materials copay for eyeglass lenses once every 12 months and frames once every 24 months or contact lenses in lieu of eyeglasses once every 12 months - see details.

Life Insurance Plans - MBA VEBA and Minnesota Life Insurance Company offer seven unique life insurance plans allowing each bank to choose the best plan for its group of employees. Bonuses can be included at the option of the bank to Plan II, III, or IV. Click here for more information.

Additional Life Plan - MBA Insurance Group and The Standard Insurance Company offer this additional life coverage to those banks that are currently on MBA VEBA's life insurance, allowing your employees to supplement the life insurance they currently carry through you. Long Term Disability - MBA VEBA and UNUM Life Insurance Company provide a long-term disability plan with three elimination period options making it possible to tailor the plan to each bank's situation. Bonuses can be included in the coverage at the option of the bank. LTD coverage also includes UNUM's employee assistance and world-wide emergency travel assistance programs. Felonious Assault Plan - MBA VEBA and the Zurich-American Insurance Group provide felonious assault coverage that covers employee deaths and dismemberments occurring during a felonious assault. Click here for more information.

Group Accident Plan - MBA VEBA and the Zurich-American Insurance Group offer a group accident plan available to employees that provides coverage for accidental death and dismemberment. Click here for more information.

Annual Group Accident - MBA Insurance Group and the Zurich-American Insurance Group offer an individual accident plan available to individual employees and directors that provides coverage for accidental death and dismemberment. Click here for more information.

Cancer Insurance - MBA Insurance Group offers coverage designed to provide cash for the indirect costs associated with cancer or lump sum coverage for first occurence cancer.  We offer a variety of policies through Allstate, Central United Life and United Teachers Association.  Click here for more information.

Long Term Care Insurance - MBA Insurance Group and UNUM offer Long Term Care coverage for your employees, their spouses, parents, grandparents, and siblings. Coverage can be designed by each person to meet their specific needs.
